Program Description
Lifescape Community Services, Inc. continues to offer the Senior Mental Health Program to conduct outreach, identify older adults with depression, and provide them with case management services through its second year with WCCMHB funding. The Senior Mental Health program utilizes the Healthy IDEAS (HI) evidence-based program, which is proven to diminish depression and reduce suicide risk in underserved older adults. Lifescape will perform outreach, provide case management, screen for depression, and provide education about mental health for older adults and their families. Lifescape will continue to confront stigmas associated with mental health issues through education on general mental illness and depression.
Older adults will be connected to community mental health service providers for diagnosis and treatment and receive wrap-around services. These services include, but are not limited to, transportation, information and referral, family caregiver support, nutritional services, and connection to the retired senior volunteer program. These services will reduce suicide risk factors and strengthen protective factors. Senior Mental Health Program aims to serve 350 residents of Winnebago County.
