In conjunction with UI Health Mile Square Health Center (MSHC) - L.P. Johnson Rockford and the Winnebago County Justice Center (WCJC), the University of Illinois College of Medicine Rockford (UICOMR) Department of Family and Community Medicine's CARE program, will provide a fully integrated transition to care services for individuals who are incarcerated upon their return to society for medical, mental health, and substance use disorders.

Eligible participants are identified while incarerated and enrolled in the program upon returning to the community. Substance use and mental health services are available at the clinic for the general public as well.
